Forms are at the heart of how clients interact with your business and submit their information in Breely. This article provides a comprehensive overview of how forms work, from their core functionality to advanced customization and sharing options.
Where to Find Forms
You'll find the "Forms" section in the left-hand navigation panel within your Breely account.
What Are Forms Used For?
Breely forms are incredibly versatile and can be used in several ways:
Booking Appointments:
Default Booking Form: Every Breely account starts with a booking form that allows clients to schedule one-on-one appointments.
Class Scheduling: Forms can also integrate with your class booking features.
Standalone Information Collection: Use forms independently of scheduling to gather any kind of information.
Lead Forms: Collect contact details from potential clients.
Payments: Charge clients a fixed amount for services or products.
Sales: Allow clients to purchase gift certificates, packages, or subscriptions.
Mixed Use: Combine any of the above functionalities into a single form.
Navigating the Forms Section
When you click on "Forms," you'll see a list of your existing forms.
Submissions: The "Submissions" tab contains records of all completed form submissions from your clients.
Editing a Form: Click on a specific form to enter its editing interface.
Form Structure
Each form is organized into sections, each containing one or more questions. There's also a final "Thank You" page.

Thank You Page: The message clients see after submitting the form. If the form includes appointment booking, this page will also show appointment confirmation details (add to calendar, reschedule, cancel options). For standalone forms, it will only display your thank you message.
Adding and Reordering Questions & Sections
Add a Question: Click "Add a question" within a section to choose from various question types.
Reorder Questions: Click and drag questions to change their order within a section.
Reorder Sections: Use the "..." (dot menu) next to a section title to move it up or down.
Question Settings
Each question type has unique settings, but common options include:
Label: The text displayed for the question.
Required: Mark a question as required to ensure clients provide an answer before submitting.
Previewing Your Form
To see how your form will appear to clients, click the "Preview" button. This allows you to test the flow and appearance before sharing.
Advanced: Conditionals
Conditionals can be used to create dynamic forms where questions or sections appear or change based on previous answers. For example, if a client answers "Yes" to "Is this your first visit?", you could show a different set of follow-up questions.
Styling Your Forms
Customize the look and feel of your forms to match your brand:
Background: Choose a full background image or a solid background color.
Logo: Add your business logo.
Theme: Select a theme and adjust colors, fonts, and label appearance to create a cohesive brand experience.
Automations
Automations are actions that trigger after a form is submitted. These can be powerful tools for client communication and internal workflows.
Email & SMS: Send automated emails or text messages to clients (and yourself/team) after form submission or based on appointment status (scheduled, rescheduled, canceled).
Conditional Sends: Send different messages based on specific criteria within the form (e.g., send a "prep information" email two days after an appointment is scheduled).
Reminders: Set up automated reminders for appointments.
Default Messages: Breely provides default messages for common automations, which you can customize.
Sharing Your Forms
Once your form is designed and automations are set up, it's time to share it with your clients:
Standalone Link:
Copy a unique URL for your form.
Share this link on social media (e.g., Instagram profile), in emails, or anywhere else you need a direct link to your booking page.
Embed on Your Website:
Standard Embed: Embed the form directly onto a page on your website, making it visible to visitors.
Pop-up: Create a button on your website that, when clicked, triggers the form to appear as a pop-up.
Multiple Forms for Specific Needs
Breely allows you to create multiple forms, which is excellent for customizing the client experience based on the service or consultation they are booking.
Example Use Case: Create a separate form specifically for "Virtual Consultations" with questions and information relevant only to those appointments.
Copy from Existing: Save time by copying an existing form as a base, inheriting its questions and automations.
Customize Questions: Remove irrelevant questions (e.g., phone number for a Zoom-based virtual consult).
Filter Appointment Types: Configure the appointment question to only show "Virtual Consult" event types.
Custom Thank You: Provide specific instructions (e.g., "Please join the Zoom link at the time of your appointment").
